The Registration Office is located at 120 Lawrence Street, registration forms can be printed below or can be picked up at the school.  Our registration office hours are Monday- Friday 7:45am-3:45am, Summer office hours are Monday- Thursday 8:00am-3:30pm.

Below is a list of documents that are needed for registration:

• An original birth certificate or passport
• Immunization and Physical 
• Custody Papers (if applicable)
• Proof or residence ( must include name and address of parent/ guardian), please submit two items from the following list:

                                    * Lease

                                     * Mortgage Statement or closing document

                                     * Signed and Notarized Residency Affidavit

                                     * Current Utility Bill

                                     * Current Phone Bill

                                     * Current Tax Bill
